Registration Begins for the 2011-2012 School Year

The District will begin accepting registrations on Monday, November 15th for the 2011-2012 school year. Registration forms can be picked up at any school office or can be accessed on the District’s website: www.galt.k12.ca.us.  Parents are urged to register their children early to assist the District in planning for the 2011-2012 school year. Children are not required to be present at the time of registration. 

Enrollment in kindergarten requires that a child must be 4 years and 9 months of age by September 1, 2011.  The birth date must have been on, or before, December 2, 2006.  Enrollment in first grade requires that a child must be 5 years and 9 months of age by September 1, 2011.  The birth date must have been on, or before, December 2, 2005.  Children who are in kindergarten this year are automatically registered for next year's first grade.
 
THE FOLLOWING DOCUMENTS ARE REQUIRED: 

1. Birth Certificate (or photostatic copy), Notification of Birth Registration, Passport, Baptismal Certificate or Affidavit.  Affidavit forms are available at any school office.

1. Written record of immunizations for polio, measles, rubella, mumps, diphtheria, whooping cough, tetanus, varicela, and hepatitis B series.  Religious or medical exemptions are acceptable.  A medical exemption requires written verification from a physician. Parent/guardian's signature is required on the Personal Belief Affidavit for Religious Exemptions. Students who have not completed their immunization series will be considered a "conditional admittance" and must have all records completed by April 14, 2011 or be removed from class lists.  A physical exam is required for 1st grade only.  Forms are available at any school office.  Please remember the physical must be obtained within the 18 month period prior to your child beginning first grade.

1. Oral Health Assessment is required in either kindergarten or first grade, whichever is his or her first year in public school. The assessment must be within the 12 month period before your child enters school and must be performed by a licensed dentist or other licensed or registered dental health professional. Fortunately in our District we have local dentists that perform the required oral health assessment at no cost. Further information on this requirement will be provided upon registration
