Registration Information

The following documents are required for registration:

1. Birth Certificate (or photostatic copy), Notification of Birth Registration, Passport, Baptismal Certificate or Affidavit. (Affidavit forms available at any school office)
2. Written record of immunizations for polio, measles, rubella, mumps, diphtheria, whooping cough, tetanus, varicela, and hepatitis B series.  Religious or medical exemptions are acceptable, but will require a written verification from a physician.  Parent/guardian's signature is required on the Personal Belief Affidavit for Religious Exemptions.
3. Proof of residency, such as water bill, SMUD bill, or PG&E bill.

STUDENTS WHO HAVE NOT COMPLETED THEIR IMMUNIZATION SERIES WILL BE CONSIDERED A "CONDITIONAL ADMITTANCE" AND MUST HAVE ALL RECORDS COMPLETED BY APRIL 15 PRIOR TO THE YEAR OF ATTENDANCE OR BE REMOVED FROM CLASS LISTS.

Preschool:

To be admitted to preschool at Fairsite Elementary a child must be 3 years and 9 months of age by September 1.

Kindergarten:

To be admitted to Kindergarten, a child must be 4 years and 9 months by September 1.  No physical exam is required.

First Grade:

To be admitted to first grade, a child must be 5 years and 9 months by September 1.   A physical exam is required.  Forms are available at any school office.  The physical must be obtained within the 18 months prior to your child beginning first grade.  Children who are currently attending kindergarten will automatically be registered for next year's first grade.
